Teen worker raised safety concerns before fatal fall

The devastated mother of a teenage boy who fell 12 metres to his death during a building refurbishment has pleaded for tighter regulations on WA worksites, including the introduction of industrial manslaughter laws.

A MAN has died and another is fighting for life after a crane accidentally dropped 1.5 tonnes of concrete in Melbourne.

Three men were working in the pit at the basement of the residential building site when the chain holding the kibble - a bucket used to transport concrete on a work site - reportedly snapped.

On Thursday, July 19th 2018 C.A.M.P. was informed by a training centre of a functional issue when using a Giant in a simulated two-person rescue scenario.

This issue was related to the impossibility to start the rescue descent with a rescue load and it is due to a possible manufacturing defect of the internal mechanism for the batches 12 17 and 03 18.

You may have also noticed a 2D code printed on each piece of Petzl gear, it looks like a QR code. It’s not exactly a QR code, it’s actually a 2D Rectangular Datamatrix code and it contains the individual serial number of that piece of gear.

• Incident date: 23/08/2018
• Event: Fall from height through plastic roof sheeting
• Location: Tomago in Northern NSW

A 19-year-old first year apprentice electrician was on the roof of a single storey commercial building, assisting with the installation of solar panels. The worker stepped on a plastic roof sheet and fell approximately 7 metres.

The rope line had excessive slack between the worker and the anchorage point. The excessive slack in the rope line allowed the worker to fall the 7 metres to the concrete slab below.

NSW offers rebates to small businesses and sole traders who buy and install equipment that makes their workplace safer.

The small business rebate gives up to $500 back to small business owners who buy and install eligible safety items to address a safety problem in their workplace.
