Petzl Sequoia Harness Notification about Leg Loop FAST Buckle

There have been recent postings on social media regarding the Petzl SEQUOIA harness, specifically the harness leg loop FAST buckle.
In some rare cases, wearers have experienced the FAST buckle of the leg loop working itself into a partially released position.

On March 30, 2016, during routine equipment inspections, one customer detected cracks on the stainless steel arms on three ASAP LOCK units.
In light of this information, a complete inspection of Petzl ASAP LOCK inventory was conducted and no cracked ASAP LOCK arms or Side plates were found.

During a recent quality inspection on the new MGO OPEN connectors sold since January 2016, Petzl found the heat treatment on a batch of the screws used for this product to be non-compliant. The heat treatment defect may potentially cause cracks to form over time.

Call for Inspection for work harnesses with "Click-Lock" buckles: TREE CORE, CORE CL, and CORE PLUS CL made by EDELRID
Based on customer feedback, EDELRID have been alerted to the fact that the "Click-Lock" buckles for the thigh loops on the harness types TREE CORE, CORE CL and CORE PLUS CL may open if only one of the push buttons is pressed. This was caused by a temporary error in the manufacturing process of a limited number of buckles from their buckle supplier for the harness models listed below.
Please note: Even if the buckle is open, the design of the harness ensures safety in the loading direction (dropping out from it is not possible), nonetheless the error might lead to injury if the buckle on the thigh loop opens unintentionally. This buckle type is used only on the thigh loop of the said harnesses.
To rule out any risk, EDELRID is requesting all users of said harnesses to immediately stop use and inspect them per the instructions attached.

Twin tail fall arrest lanyards (interim advice)
A worker recently received fatal injuries as a result of falling from a transmission tower near Toowoomba. The worker was using a twin tail fall arrest lanyard at the time of the incident when he fell. He suffered fatal injuries when his lanyard failed.
