Inquest into the Death of Shaun McBRIDE

State Coroner of the Coroner's Court of Western Australia has delivered his inquest into the Death of Shaun McBRIDE, an Irish scaffolder who fell from a wharf in Dampier WA 6 years ago.

The inquest states recommendations to the Australian Standards for the following ammendments:

"I recommend that the committees responsible for the relevant Australian Standards consider amendments to ensure that people working over or adjacent to water or liquid who may be at risk of falling into the water and drowning wear an approved PFD – including AS/NZS 1891.4:2009 (committee SF-015) and AS/NZS 4576:1995 (committee BD-36)."

Gas kills worker in manhole due to lack of proper confined space training

A 30-year-old man, working on contract with Jusco's health and sanitation wing, died of asphyxiation in Jamshedpur on Tuesday morning after his supervisor allegedly sent him down a 20-feet-deep sewer manhole on the company's premises to fetch a gas measurement equipment that the latter accidentally dropped.

Another contractual worker, Aakash Karua (31), who had followed Ganesh into the manhole also suffered methane poisoning, but was rescued on time. He is being treated at Tata Main Hospital (TMH), where doctors said his condition was stable.

The construction industry is the most dangerous for ladder use, with falls from ladders being the most common causes of fall-related injuries and fatalities, according to data in a 2013 Safe Work Australia (SWA) 2013 falls from height report.

When a fatality occurs, head injuries are the most common cause of death, with 76 per cent of the 112 workers who died from a fall from a height in the four years 2008–11 doing so because of a head injury.

A husband, wife and brother were found dead inside a water tank on a rural NSW property. They were killed yesterday after entering a tank that had filled up with poisonous fumes on their property in Gunning, around 60km north of Canberra.
